Please see communication from the NHS regarding future vaccinations for Year 9 students in June.

Dear Parent/Carer

We are coming to your child’s school in June 2024 to vaccinate the YEAR 9 pupils Diphtheria, Tetanus and Polio (DTP) and Meningitis (Men ACWY).

How to give consent.

  • Go to the website below or click on the following link. This will take you to the online teenage booster vaccination form.

https://www.leedsimmunisations.co.uk/Forms/DTP

  • Enter the unique school code for your child’s school. The code for Co-op Priesthorpe is EE144588 (please ensure you enter all letters and numbers).

  • The code for West SILC pupils in attendance at Co-op Priesthorpe is EE134884

Important: If you do not wish your child to receive the vaccinations, you are still required to complete the “refusal section” on the form so that we understand your wishes. If we do not receive a consent or refusal form, your child will be given an opportunity to consent for themselves on the day after being assessed by a nurse as Gillick competent to do so.

PROTECTING YOUR CHILD AGAINST SERIOUS DISEASES

In year 9 your child is offered two vaccinations as part of the UK routine immunisation schedule. These are the Meningitis ACWY vaccine which helps to protect against Meningitis and Septicaemia, and the DTP (teenage booster) which helps to protect against Diphtheria, Tetanus and Polio. Your child may also be offered the MMR vaccine to protect against Measles, Mumps and Rubella if they did not receive this when they were younger.

Your child will bring a confirmation slip home when they have received their immunisation(s).

For further information about these vaccinations go to the www.gov.uk website and search “immunisations for young people” or click here
